Cardio Pulmonary Rehabilitation

Following this detailed assessment, we will develop a personalized, medically-supervised program designed to address the unique needs of each individual, focusing on symptom management and enhancing overall quality of life.

Cardiac Rehabilitation

Patients recovering from various heart-related conditions like angina, heart attack, heart valve repair or replacement, stenting, and heart failure receive individualized care designed to enhance cardiovascular health.

Participants in the Cardiac Rehabilitation program will learn to:

  • Safely engage in exercise as prescribed by their physician
  • Recognize their personal risk factors for heart disease
  • Understand how to reduce these risk factors to avoid future cardiac events
  • Receive support for quitting smoking
  • Get guidance on nutrition and weight control
  • Monitor and manage blood pressure through diet, exercise, and medication
  • Identify any necessary behavioral changes

Supervised Exercise Therapy (SET) for Peripheral Arterial Disease

The Supervised Exercise Therapy Program is a comprehensive program designed to assist patients in exercising safely to achieve the goals of increasing activity without pain, improving blood flow to the lower extremities, to identify risk factors for peripheral arterial disease and how to decrease these risk factors.

The SET Program staff, in consultation with your referring Provider, will develop a treatment plan that will be individualized to meet your specific needs. You will be provided with instructions on how to begin and gradually increase your exercise to achieve set individualized goals to help improve your Peripheral Vascular Disease and symptoms.

Patients in the SET program will receive and learn to:

  • Safely engage in exercise as prescribed by their physician
  • Recognize their personal risk factors for vascular disease
  • Understand how to reduce these risk factors to avoid further vascular disease
  • Receive support for quitting smoking
  • Get guidance on nutrition and weight control
  • Identify any necessary behavioral changes
  • Develop goals to achieve positive, healthy outcomes

Pulmonary Rehabilitation

Patients with lung conditions like COPD, interstitial lung disease, and emphysema receive personalized support from a team of experienced registered respiratory therapists.

Participants in Pulmonary Rehabilitation the program will receive:

  • Education and support on various topics such as diagnosis, nutrition, stress management, breathing exercises, quitting smoking, oxygen therapy, medication use, preventing exacerbations, creating action plans, providing psycho-social support, planning for end-of-life care, exercise training, and post-program guidance.
  • Assistance to those dealing with depression, anxiety, or other emotional challenges related to their chronic illness
